After my puppy finishes runs around or exercises rigorously for about 15 minutes or so, he will cough, almost as if he is gagging. Could this be an indication of a respiratory or heart problem?

Possibly, please make an appointment with your vet, who depending on his/her findings, may recommend a cardiologist.
A young pup ( you don't say how old) should be able to play hard without coughing and gagging.
