Here is the trouble with cat claws... they walk on them non-stop so it is almost impossible to get a cat to rest it's paw when it should be healing. It sounds like it may be infected from what you are describing. Was the medication given twice daily as prescribed or were any doses missed? Since cats walk in their litterbox where the urinate/defecate and walk on the ground they are susceptible to all types of bacteria - even some that may be resistant to our commonly used antibiotics. What I would recommend is calling your vet - they will likely tell you to come it and have it rechecked. I see no reason for your vet to charge you another $125 since it is a recheck appointment. (To tell you the truth, that is a very inexpensive price for the procedure and medications - some clinics would've charged $300 or so!) Anyhow - they will likely switch antibiotics (Clavamox drops are good or they can even give her an antibiotic injection called "Convenia" which is time released and just as good as the oral ones).
Since I can't see it, I can't tell you exactly what is happening but I would think that an antibiotic and an anti-inflammatory medication like Metacam would be what I would recommend (all available at your vet). I can't tell you what their cost is b/c all clinics are different, but I don't see it being more expensive since they won't likely be doing any procedures.
The other thing you can do at home is do not allow her to jump off of the bed, counters or anything until this is healed. The force of landing on her foot is enough to cause extreme inflammation/swelling which also may be why it appears to not be healing.
Bring her in to see your vet - they've done everything right, your cat may be licking at it (it will definitely get infected if she does this!), jumping and making it difficult for it to heal.
Best of luck - Allison Williams, DVM
