max has not eaten in 3 days, regurgitates water that he drinks and has developed an unpleasant body odor. what could be wrong?

It's difficult to specify the cause but it is potentially serious. 3 days without food and not keeping down water will lead to dehydration. This could be a primary gastrointestinal upset due to toxins or infection or it could be secondary to another illness. I think it would be important to check out his organ function with a blood test as he is fairly elderly. The body odour can be simply due to not washing himself because he feels sick, but also if they body is not clearing toxins as it should, this can cause an unpleasant smell. I recommend that you take him to the vet for a physical exam if you can.
