Try hanging a small bell at your door.  You can get "jingle bells" this time of year.  Hang it from the door with a piece of yarn and make it at your dog's nose level.  Whenever you take him out, hold his nose and ring the bell with it.  Usually dogs learn very quickly that when they ring the bell with their nose, you will take them out.  This way, you can hear the bell from somewhere else in the house and know he needs to go.  In regards to his food, two meals a day is appropriate for a four month old.  However, 4 cups of food per day sounds like a lot.  Look at the back of your dog food bag and there will be a chart detailing how much food your puppy should get based on his age and weight.  If you have any other questions, you may contact me or your veterinarian.
