A ravenous appetite in a cat that either has lost weight or does not gain weight is a big red flag for hyperthyroidism (an over active thyroid). This is easily diagnosed with a physical exam and bloodwork. Most cases respond very well to medication. Hyperthyroidism can lead to heartproblems if left untreated. Other maldigestion problem can lead to weight loss despite a good appetite, but thyroid disease is going to be the most common cause.
