HI there. Your dog sounds like he is constipated, either due to dietary problems or an obstruction in his rectum/anus area.  The swelling around the anus may be what is called a rectal prolapse, where the inside of the rectum gets pushed out due to excessive straining, or an anal sac problem like and abscess. Anal sacs are present in all dogs on both sides of the anus and can become blocked, infected and form an abscess which is very painful. These are the most likely causes of the problem in a dog his age, but other things are also possible. Either way, I would take him to a vet as soon as you can, so that he can be given pain relief as well as to find a diagnosis. If it is a rectal prolapse, the prolapsed tissue can become necrotic (death of tissue due to lack of blood supply) or infected. Also he can develop dilation of the colon as a secondary problem if he is not treated soon. Hope this helps and good luck!

Anal sac abscesses are quite common in dogs and they usually require surgery, which is most cases is successful. A prolapse may be able to be treated with a very minor procedure but more severe cases may need surgery. As I said, there are other possibilities as there are many structures in that area that can swell but these are the most likely. Hope that answers your question.
